Documentation checklist - skilled workers
Below is a list of the documents you will need if you are selected for Queensland nomination and invited to apply through any of these 4 pathways.
Please note that we will only consider current versions of forms, which are available in your document portal. These documents are the minimum requirement to support your Queensland application. If required, we may request more information or supporting documents.

Document checklist explanatory notes
- State Nomination Application Form – this form is only available when you receive an invitation from Migration Queensland. Please make sure all the required questions are answered and the form is signed, so we can assess your application.
- Passport bio page – a high-quality colour scan of your passport bio page. The passport number must match the passport number provided in your Registration of Interest (ROI).
- VEVO statement – a copy of your recent Visa Entitlement Verification Online (VEVO) statement. VEVO allows a visa holder to check their visa conditions and validity.
- Skills assessment or Evidence of Registration – a valid skills assessment for your nominated occupation. Some occupations must provide Evidence of Registration instead. Doctors, GPs, Resident Medical Officers and medical specialists will need to provide their current AHPRA registration. Solicitors will need to provide their Certificate of Admission.
- Evidence of residential address – a copy of your current rental lease or a recent utility bill (issued within last three months) that includes your name and address. If this cannot be provided, please provide a statutory declaration stating your current residential address.
- Employment contract – complete employment contract(s) for all employment in Queensland within the required Queensland work experience period. The contract must show you are working at least 20 hours a week in your nominated occupation (or closely related role). The work experience requirement is calculated from the time you submit your ROI. Contracts for employment that are unrelated to your nominated occupation do not need to be provided (e.g., food delivery work).
- Statement of Service form – this form is to be completed by the employer/s of onshore applicants as evidence for the Queensland work experience requirement. If you have had more than one employer during the period of required work experience, each employer will need to complete this form. If you are employed through a labour hire company, your Statement of Service must come from the labour hire company.
- Evidence of work experience – at least 3 payslips to demonstrate the required period of work experience in Queensland (3 months for Building & Construction, 6 months for subclass 491 and 9 months for subclass 190). Payslips should cover the beginning, middle and most recent period of employment. Work experience requirements are counted from the date of your ROI submission.
- Evidence of ROI annualised earnings – your ATO Income Statement from the time you submitted your ROI. The statement must be from the time you submitted your ROI and not the time of application. Statements are required for each employer you used to calculate your ROI annualised earnings. This statement can be found in the ATO profile in your MyGov account.
- Letter from employer – offshore applicants must provide reference letters as evidence for all work experience related to your occupation on your EOI. You must also provide a letter from your current employer. If these letters are not in English, a translated copy must be provided as well.
- Current payslip – offshore applicants must provide their most recent payslip from their current employer to show they are still working in their nominated occupation
- Graduation Certificate or course completion letter – a copy of your Graduation Certificate, academic transcript or letter of completion from your Queensland university that states your degree name, study location and date of graduation. You must have graduated after 1 July 2021 to be eligible for the Queensland Graduates pathway.

Evidence of ROI annualised earnings | Profit and Loss statement - provide the Profit and Loss statement used to calculate your ROI annualised earnings. The statement must clearly show that it was produced and signed by a registered accountant. Business Activity Statements (BAS) - provide your latest BAS from your business’ online account with the Australian Taxation Office (ATO). |
